📊The analysis of the 2024 Brazilian Transplant Registry reveals a critical finding: despite an increase in the absolute number of transplants, the donation effectiveness rate remained at only 27%, below the national target of 30%, mainly due to a high family refusal rate, which reached 46% of all interviews conducted. This mismatch directly compromises system performance, limiting growth in transplants per million population, even with an adequate potential donor notification rate (71 pmp).

From a technical standpoint, this result highlights a structural bottleneck in the conversion of potential donors into effective donors, reinforcing the need for qualified interventions in family approach strategies, professional training, and strengthening of Organ Procurement Organizations.
